Site Risk Assessment Template
A risk assessment tailored to a specific site or location. It records the site details and any permits or notices issued, describes each hazard and its potential consequences with the control measures and risk rating, and captures who is affected and any feedback or additional controls required.
The Management of Health and Safety at Work Regulations 1999 require risks to be assessed wherever work is carried out, including at sites away from a fixed base. This form gives you a practical, site-level way to meet that duty and supports the operational control requirements of ISO 45001.
Built for ISO 45001 and Site-Level Assessment
The assessment supports the hazard identification and operational control requirements of ISO 45001 (clauses 6.1.2 and 8.1):
- Site details recorded together with any permits, notices or documents issued, by whom and when
- Each hazard and its potential consequences described, with the control measures and a risk rating
- The people affected or at risk clearly identified
- Space for feedback from affected parties and any additional controls required
What's Included in this Site Risk Assessment
The form is set out so a site can be assessed and signed off in one place:
- Header - completed by, date, assessment number and location
- Site documents - details, type and issued by / date for permits, notices or related documents
- Hazard assessment - description of each hazard and potential consequences, control measures and risk rating
- Affected persons and notes - who is at risk, feedback from affected parties and additional controls required
Who Needs a Site Risk Assessment?
Any organisation whose people work at customer sites, project sites or locations away from a fixed base - construction and trades, facilities, installation, maintenance and service teams. It is a core record for any business working towards or maintaining ISO 45001 certification.
Included in the ISO 45001 Toolkit
This form is part of the ISO 45001 Toolkit - risk assessments, registers, procedures and guidance for managing health and safety in one download - and is included in every alphaZ ISO toolkit that covers ISO 45001.
Pairs with the General Risk Assessment
Use the F-HS20 General Risk Assessment for activity-based assessments, and the F-HS14 Task Risk Assessment for a single task, then track everything in the F-HS12 Risk Assessment Register.
