Combined Driver FORS Declaration Form Template

The F-FORS2 Driver FORS Declaration - Combined is a Word form that brings the driver's declaration of responsibilities together with their collision history, driving endorsements and a set of health questions, so each driver completes and signs a single form.

It lists the same FORS (Fleet Operator Recognition Scheme) policies, documents and risk assessments as F-FORS1, and is classified as confidential because it holds health information about the driver.

What's Included in this Form

  • Driver responsibilities - the documents each driver needs to be aware of
  • FORS policies - health and safety, complaints, transport infringements, serviceability and roadworthiness, load safety, tyre management, driving standards, staff induction, in-vehicle communication, health and eyesight, working time and drivers' hours, fuel, emissions and air quality, road traffic collisions, passenger safety where relevant, specialist operations, operational security and counter terrorism
  • Other documents - the Highway Code, the FORS Manual and FORS guidance, company handbooks including the drivers handbook, the employment contract, and the company management system with its policies, forms and procedures
  • Risk assessments - lone working, manual handling, hazardous substances, driving and deliveries, slips, trips and falls, loading and unloading, reversing, manoeuvring and turning, access to vehicles and working at height, working around moving vehicles, coupling, uncoupling and towing, and in-vehicle technology
  • Collisions - the date and details of any collision, at work or off duty, that involved an insurance company or the police, whoever caused it
  • Endorsements - the date and details of any driving convictions, licence endorsements, fines, driving infringements or restrictions, on or off duty
  • Health - yes or no questions on concerns about vision or fitness to drive, blackouts, fits or epilepsy, medication or treatment that could affect driving, other medical conditions, heart conditions and sleep disorders, with space for further details

The Declaration

The driver declares that they understand their responsibilities and the company driving policy, that the information given is complete and accurate, and that they feel fit and safe to drive. They agree to report any health or eyesight condition that affects their driving, every accident or incident involving a company vehicle, and any endorsements, infringements or restrictions, and they authorise the company to check their driver record with the DVLA or an approved intermediary.

Who Needs a Combined Driver Declaration?

Fleet operators holding or working towards FORS accreditation that want one signed form per driver covering responsibilities, collision and endorsement history and fitness to drive, rather than separate declarations.

Included in the FORS Document Compliance Toolkit

This form is part of the FORS Document Compliance Toolkit, alongside the FORS policies, the drivers handbook and the FORS registers.

Using the Form

Edit the lists to match the policies and risk assessments your drivers work to, ask each driver to complete and sign the form, and ask for a new one when anything changes. Store completed forms securely, with access limited to the people who need them, as they hold health information.
