FORS Operational Performance Action Plan Template
The F-FORS3 Operational Performance Action Plan is a Word form for reviewing how a fleet has performed over a set period and planning the targets for the next one. It records who completed it, the date and the time period covered.
It was written for operators working to FORS (the Fleet Operator Recognition Scheme), where performance data and targets are reviewed regularly, and is classified as confidential.
What's Included in this Form
- Review of performance - the performance achieved for total distance travelled, total road traffic collisions, total injuries, reportable injuries, and total fines and penalty charges
- Review of targets previously set - each target with a review of how far it was achieved
- New targets - each target with the actions or activities needed to achieve it, and the data capture method, the monitoring and how success will be measured
Who Needs an Operational Performance Action Plan?
Fleet operators holding or working towards FORS accreditation, and transport managers who want a simple record of fleet performance and the targets agreed for the period ahead.
Included in the FORS Document Compliance Toolkit
This form is part of the FORS Document Compliance Toolkit, alongside the FORS policies, the drivers handbook and the FORS registers.
Using the Form
Choose the review period, enter the figures for each performance measure from your own records, and review each target set last time. Then set the new targets with the actions behind them and how they will be measured. The performance measures can be changed to suit your operation.
Performance Measures on the Form
- Total distance travelled
- Total road traffic collisions
- Total injuries
- Reportable injuries
- Total fines and penalty charges
Setting the Targets
Targets are easier to review when each one is specific and measurable, such as fewer collisions or penalty charges than the period before, and has a named person behind the actions. The data capture column on the form is there to record where the figure will come from, so the next review uses the same source.
Document Control
The footer carries the form number and title, the issue number, page numbering and the information classification, Confidential, so completed forms can be controlled and filed with the rest of your FORS records.
Where the Figures Come From
Injury figures can be taken from the ER18 Accident Statistics Register, vehicle and fuel records are kept on the ER20 FORS Vehicles Register, and fuel and emissions targets can be tracked on the ER23 Fuel Emissions Register.
Related Documents
The F-IMS33 FORS Review Register covers the wider FORS checks, the F-FORS4 Communications Planner plans the driver briefings that support the targets, and company objectives can be kept on the F-Q11 Company Objectives form.
